Completing the necessary paperwork in advance helps to shorten the time of the first office visit:

For new patient, please fill out the forms in the New Patient Registration package and bring the

forms with you. The forms can be filled out online and then print out and signed.

If your insurance covers acupuncture treatment, please gather the information on this Insurance Verification Template and bring the printed template as well as your insurance ID card with you.

For new facial rejuvenation acupuncture patient, please fill out the forms in the Facial Patient Registration package and bring them with you.
